Social Work Care Coordinator

We are seeking a highly skilled Clinical Social Worker to join our interdisciplinary team. In this role, you will be responsible for assessing and developing individualized social work care plans that address the psychosocial, financial, community resource, long-term planning, and bereavement needs of our patients and their families.
The ideal candidate will have a Master's degree in Social Work or a related field and at least one year of experience in a healthcare setting. A licensed social worker in the State of New Mexico is also required.
Your responsibilities will include:
- Assessing patient needs and developing individualized care plans
- Collaborating with the interdisciplinary team to provide comprehensive care
- Providing counseling and support to patients and their families
- Advocating for patients and their families within the healthcare system
We offer a competitive salary range of up to $32.17 per hour, depending on experience. Our benefits package includes health insurance, retirement savings plan, paid time off, and opportunities for professional growth and development.
