Physician or Advanced Practitioner

We are seeking a dedicated Physician or Advanced Practitioner to join our team at LifeLong Medical Care, providing high-quality care to patients in our Homeless Services Department.
The ideal candidate will be committed to building a collaborative team model of care and have experience working with underserved populations.
As a member of our team, you will have the opportunity to work in a variety of settings, including health centers, skilled nursing facilities, and supportive housing sites.
Responsibilities- Provide preventive, urgent care, and primary care health services to patients in our Homeless Services Department.
- Conduct clinical assessments, order diagnostic evaluations, and plan programs of care for patients.
- Provide verbal and written education and counseling to patients, families, and caregivers.
- Maintain patient records necessary for clinic reporting, billing, and quality improvement.
- Participate as a member of the site clinical services team and attend clinical team meetings.
- Provide specialty care services, clinical case management, and team participation to designated programs.
- Coordinate care with other health team members, including case management and behavioral health staff.
- Act as an advocate for patients and liaison with outside agencies as appropriate.
- Commitment to providing primary care services for underserved populations with demonstrated ability and sensitivity in working with diverse populations.
- Ability to take initiative, work independently, and make sound judgments within established guidelines.
- Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds.
- Strong organizational, administrative, multi-tasking, prioritization, and problem-solving skills.
- Ability to work effectively under pressure in a positive, friendly manner and to be flexible and adaptive to change.
We offer a competitive salary range of $197,000-$218,000/year for MD/DO and $123,000-$146,000/year for APP, pro-rated depending on years since licensed, as well as a sign-on bonus and excellent benefits, including medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs.
LifeLong Medical Care is a qualified site for federal and state loan repayment programs (HPSA score is 21).
