Job Description: JobRialto seeks a High-Acuity Patient Care Manager to oversee the delivery of critical care services. As a High-Acuity Patient Care Manager, you will lead interdisciplinary teams to ensure comprehensive patient outcomes, working closely with clinicians to develop and implement effective care plans.

This is a leadership role that demands strong clinical knowledge, strategic thinking, and excellent communication skills. You will collaborate with colleagues to identify areas for improvement, implementing changes to enhance patient care and safety.

Key Responsibilities:
  • Lead interdisciplinary teams to deliver comprehensive patient care.
  • Develop and implement effective care plans in collaboration with clinicians.
  • Collaborate with colleagues to identify areas for improvement and implement changes.

Estimated Salary: $90,525 - $135,743 per year (based on location and experience).

Requirements:
  • Active Texas RN license
  • Minimum of 3 years of current ICU experience, preferably in a Long-Term Acute Care Hospital (LTACH) setting
