Key Responsibilities- Provide direct patient care in the home setting, including assessment, planning, implementation, and evaluation of care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Implement evidence-based practices to improve patient outcomes and quality of care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Graduate of an accredited nursing program, with a Bachelor of Science in Nursing required within 5 years of hire.
- 1 year of relevant experience as a Registered Nurse.
- Licensed to practice as a Registered Nurse in the Commonwealth of Virginia.
- American Heart Association (AHA) Health Care Provider BLS certification.
- Valid Virginia Driver's License.
We offer a competitive salary range of $82,000 to $110,240 per year, depending on experience. Additionally, we provide a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
