Nurse Care Coordinator

We are seeking a skilled and compassionate Registered Nurse to join our team at The Family Support Center. As a Registered Nurse, you will play a vital role in promoting individual health goals and providing high-quality care to our clients.
Key Responsibilities:- Evaluate and identify individual care requirements on a continuing basis.
- Complete health reports weekly and review health tracking and medical records.
- Ensure that universal precautions are implemented and maintained.
- Administer medications and treatments as prescribed.
- Review medication EMARS/TARS to ensure accurate transcription and administration.
- Reconcile medications monthly and dispose of outdated or discontinued medications.
- Inspect medication storage areas for compliance.
- Collaborate with the IDT to ensure medical recommendations are carried out and brought to resolution.
- Meet with the Manager/Medical Coordinator/IDT team weekly to communicate and implement recommendations.
- Confer regularly with medical providers, including pharmacy and nutrition, as needed.
- Create and update care plans for all diagnoses, medical, adaptive, and assistance care.
- Initiate training for unlicensed personnel and review and update diagnosis lists and immunizations in electronic health records.
- Prepare training materials and schedules continuing education and in-services for DSP.
- Certify AMAP on completion of initial training and annually thereafter.
- Attend and participate in agency meetings as required.
- Carry an agency cell phone and be available to all AMAP/DSP for triage.
- Complete other tasks delegated/assigned by direct supervisor.
- Bachelor or Associate Degree in Nursing.
- 1-2 years clinical experience.
- Must hold an unencumbered license for NY State issued by the New York State Department of Education as a registered nurse.
- Background in home health, developmental disabilities, or mental health preferred.
Pay Rate: $56.00 per hour / Part-Time
