Nurse Care Coordinator

West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community-based organization dedicated to providing safe, affordable housing with supportive services. We are seeking a skilled Nurse Care Coordinator to join our team at Euclid Hall.
The successful candidate will provide a range of health coordination services to residents, enabling them to maintain their independence and dignity. This role requires a strong understanding of healthcare coordination, medication management, and crisis intervention.
Key Responsibilities- Assist residents in maintaining optimum health by observing and assessing their physical and mental well-being.
- Ensure high-risk residents receive extra assistance with health education, care coordination, and follow-up.
- Track healthcare compliance and schedule care coordination conferences as needed.
- Collaborate with the Federally Qualified Health Center on-site to provide seamless patient care coordination.
- Provide workshops and training on health, nutrition, and wellness topics.
- Coach staff on best practices in healthcare coordination and medication management.
- Coordinate with internal and external care teams to address resident medical needs.
- RN or LPN with 1-3 years of experience in healthcare coordination.
- Current NYS license required.
- Experience working with individuals with serious mental illness preferred.
- Bilingual (English/Spanish) language ability is helpful.
