Job Summary

We are seeking an experienced Hospital Social Worker to join our team. The successful candidate will be responsible for cross continuum care coordination, psychosocial assessments, and psychosocial interventions. This role involves identifying opportunities to continually improve patient care, services, and population health.

Key Responsibilities
  • Perform cross continuum care activities in various settings, including clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care.
  • Provide exceptional customer service to patients, families, physicians/APPs, and care team members, ensuring smooth transitions of care.
  • Model and orchestrate consistent, professional communications with colleagues and staff, maintaining proactive communication patterns.
  • Delegate responsibilities appropriately while remaining accountable for outcomes.
Requirements
  • Masters degree in Social Work from an accredited institution.
  • Licensed in the state of Washington.
  • Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
  • Minimum three years of experience in hospital or equivalent settings.
  • Health industry knowledge preferred.
  • Leadership experience a plus.
