We are seeking skilled and compassionate Licensed Practical Nurses (LPNs) to join our team in providing high-quality care to adult and pediatric patients in the comfort of their own homes.

Key Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • Perform focused interviews to identify specific patient needs.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
  • Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.

Requirements:

  • Current Licensed Practical Nurse (LPN) license in the state of practice.
  • Current Health Certificate.
  • Current PPD or Chest X-Ray.
  • Current BLS card.
  • One year prior LPN experience preferred.
