The RN Case Manager at Carle Health plays a vital role in coordinating the clinical care of individuals. This position requires a skilled nurse to integrate and manage the healthcare needs of patients, ensuring seamless transitions across the healthcare continuum.
Key Responsibilities:
* Screens adult Medical Surgical In-patient and observation patients, assessing their health status and identifying needs for referrals to multi-disciplinary services.
* Prioritizes patients for care coordination based on defined criteria, ensuring timely and effective care delivery.
* Develops and implements an effective discharge plan for complex care patients, working with internal and external resources to coordinate a safe transition to the appropriate level of care.
* Leads and participates in daily rounds, planning delivery, and evaluation of patient-focused care for prioritized patients.
* Documents the case management plan, including clinical needs, barriers to quality care, and effective utilization of resources.
Requirements:
* Bachelor's in Science of Nursing
* Registered Professional Nurse (RN) License in state of Illinois upon hire
* Basic computer knowledge using word processing, spreadsheet, email, and web browser
