ED Social Work Case Manager

The ED Social Work Case Manager is responsible for identifying, assessing, planning, and coordinating options and services across the continuum of care for individuals in assigned case loads. This role involves coordinating healthcare services on behalf of the patient to promote the delivery of care at the appropriate level of care, optimizing clinical outcomes and satisfaction with services.
Key Responsibilities
- Identifies, assesses, plans, and coordinates options and services across the continuum of care for individuals in assigned case loads
- Coordinates healthcare services on behalf of the patient to promote the delivery of care at the appropriate level of care
- Assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation
- Assesses and intervenes to address complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans
- Coordinates with the healthcare team to facilitate and expedite the best outcomes for the patient and physician caring for that patient
Requirements
- Bachelor's degree in Social Work
- 3 years of experience caring for patients
- Broad knowledge of chronic illness and medical and community resources
- Working knowledge of a clinical EMR system and Microsoft Office applications
- Demonstrated knowledge and skills necessary to provide services to the assigned patient population
- Flexible work schedule, including weekend, holiday, and on-call rotation
Preferred Qualifications
- Master's degree in Social Work
- Care management experience with chronic, complex illnesses in adult and/or pediatric populations
