JVM Solutions, a leading healthcare provider, is seeking an experienced Nurse Case Manager to join our team in San Antonio, TX.

Job Description

We are looking for a skilled and compassionate Nurse Case Manager to provide patient-centered care and coordinate services for eligible beneficiaries of the Military Health System. The ideal candidate will have a strong background in case management, excellent communication skills, and the ability to work collaboratively with multidisciplinary teams.

Key Responsibilities:

  • Develop and implement comprehensive care plans that address the unique needs of each patient
  • Coordinate and procure necessary equipment and supplies for patient discharge
  • Collaborate with hospital staff to ensure seamless transitions between care settings
  • Monitor patient progress and document assessments to achieve desired health outcomes
  • Identify and engage community resources to support patients' health and wellness

Requirements:

  • Three years of experience as a registered nurse case manager within the last four years
  • Associate's degree in nursing from an accredited institution
  • Certification in case management or a related field (e.g., CCM, CDMS, CRRN)
  • Current, active license to practice as a registered nurse
