Support Services Coordinator - St. Luke's University Health Network

Become a vital member of our dedicated team at St. Luke's University Health Network as a Support Services Coordinator for individuals with Intellectual Disabilities and/or Autism. Our mission is to deliver individualized support and services that empower individuals to reach their goals and engage actively within their communities.

Key Responsibilities:

  • Work collaboratively with individuals and their families to assess needs and formulate tailored support strategies.
  • Oversee and evaluate services to ensure alignment with the individual's objectives and requirements.
  • Develop Individual Support Strategies that encapsulate the individual's aspirations and choices.
  • Track the individual's development, well-being, and safety to provide continuous assistance.

Qualifications:

  • Bachelor's Degree in Sociology, Social Work, Psychology, or a related discipline.
  • Competence in computer applications.
  • Excellent organizational and time management abilities.
  • Capability to fulfill the physical demands of the role.

Work Schedule:

  • Full-Time position with adaptable 8-hour shifts.
  • Monday to Friday workweek with the possibility of a hybrid work model.

Additional Information:

  • A valid Driver's License and a clean driving record are mandatory.
  • Willingness to travel for off-site meetings and evaluations is essential.
