Job Summary

We are seeking a highly motivated and compassionate Residential Case Manager to join our team at Veterans Community Project. As a key member of our housing services team, you will be responsible for providing supportive housing services to homeless Veterans and their families, formulating case plans to promote permanent housing readiness, and working collaboratively with our team to ensure the success of our clients.

Key Responsibilities
  • Conduct initial interviews and background checks for potential residents
  • Complete an initial needs assessment and act upon critical needs appropriately and immediately
  • Orient clients to VCP programs, requirements, and expectations
  • Develop and implement individualized client-driven case plans with short-term and long-term goals
  • Monitor progress towards goals in regularly scheduled weekly sessions
  • Evaluate and adjust case plans as needed
  • Provide written warnings with consequences if satisfactory progress is not being met
  • Empower clients to become involved in their own planning and goal setting
  • Refer clients to appropriate resources to assist with meeting goals
  • Maintain accurate and up-to-date client files
  • Report critical incidents immediately to the Veterans Support Services Director
  • Collect and submit data necessary to meet funding requirements and statistical reports
Requirements
  • Bachelor's degree required
  • Two years experience providing direct care to individuals in a social service setting
  • Strong passion for the mission of VCP
  • Excellent oral and written communication skills
  • Ability to work collaboratively with a team
  • Strong performance management and evaluation capabilities
