Job Summary:

Prairieland Service Coordination, Inc. is seeking a highly skilled and dedicated Case Manager to join our team. As a Case Manager, you will play a critical role in assessing individual needs and ensuring the provision of necessary services to individuals with developmental disabilities.

Key Responsibilities:

  • Service Coordination: Assess individual needs and develop personalized service plans to ensure the provision of necessary services.
  • Communication and Collaboration: Communicate effectively with individuals, families, and service providers to ensure seamless service delivery.
  • Service Monitoring and Advocacy: Monitor services provided and advocate on behalf of individuals to ensure their needs are met.
  • Problem-Solving and Consultation: Provide problem-solving consultation with provider agencies to resolve service delivery issues.
  • Documentation and Reporting: Maintain accurate and up-to-date records and submit reports as required.
  • Team Collaboration: Participate in team meetings and contribute to the development of service coordination strategies.

Requirements:

  • Education: Bachelor's degree in human services or a related field.
  • Experience: One year of cumulative experience providing direct service to individuals with developmental disabilities.
  • Skills: Excellent communication and problem-solving skills, ability to work effectively in a team environment.

Working Conditions:

  • Work Schedule: Monday through Friday, 8:00 am to 4:30 pm.
  • Work Environment: Office setting with occasional travel to community-based settings.
