Job Title: Case Manager

We are seeking a highly skilled and dedicated Case Manager to join our team at Prairieland Service Coordination, Inc. in Decatur, IL.

Job Summary

The successful candidate will be responsible for assessing individual needs and ensuring the provision of necessary services through communication, service coordination, linkage with provider agencies, service monitoring, and individual advocacy.

Key Responsibilities
  1. Assess individual needs and develop personalized service plans to ensure the provision of necessary services.
  2. Communicate effectively with individuals, families, and provider agencies to ensure seamless service delivery.
  3. Coordinate services and monitor progress to ensure individuals receive the support they need.
  4. Advocate on behalf of individuals and families to ensure their rights and interests are protected.
  5. Maintain accurate and up-to-date records of individual service plans and progress.
  6. Participate in regular team meetings and training sessions to stay informed about best practices and new developments in the field.
Requirements
  1. Bachelor's degree in human services or a related field.
  2. QIDP eligibility as established by the Department of Human Services.
  3. Excellent communication and interpersonal skills.
  4. Ability to work independently and as part of a team.
  5. Flexibility to work hours outside of the normal workweek.
  6. Reliable transportation and a valid Illinois driver's license.
