Job Summary

We are seeking a skilled and dedicated Disease Surveillance Nurse to join our team at the City of Gary. As a key member of our public health department, you will play a critical role in investigating and responding to reported cases of communicable diseases.

Key Responsibilities
  • Investigate and provide necessary follow-up services for reported cases of communicable diseases
  • Collaborate with healthcare providers, laboratories, and other stakeholders to gather information and coordinate responses
  • Develop and implement strategies to prevent and control the spread of infectious diseases
  • Provide education and training to healthcare providers and the public on disease prevention and control
Requirements

To be successful in this role, you will need:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) credentials
  • Previous knowledge of Indiana State Department of Health disease reporting requirements desirable
  • Experience with ISDH INEDDS software preferred
