Job Summary

We are seeking a highly skilled Disease Surveillance Nurse to join our team at the City of Gary, Indiana. As a key member of our public health department, you will be responsible for investigating and providing necessary follow-up services for reported cases of communicable diseases.

Responsibilities
  • Investigate and track reported cases of communicable diseases
  • Provide necessary follow-up services to affected individuals and communities
  • Collaborate with healthcare providers, laboratories, and other stakeholders to ensure accurate and timely reporting
  • Develop and implement strategies to prevent and control the spread of infectious diseases
  • Stay up-to-date with the latest disease reporting requirements and guidelines from the Indiana State Department of Health
Requirements
  • LPN/RN credentials
  • Previous knowledge of Indiana State Department of Health disease reporting requirements desirable
  • Experience with ISDH INEDDS software preferred
