Aveanna Healthcare is seeking a skilled and compassionate School Case Nurse - LVN to join our team in providing high-quality care to medically fragile patients in the school setting.
Key Responsibilities- Implement physician-prescribed plans of care to ensure optimal patient outcomes
- Conduct patient assessments and coordinate care with the healthcare team
- Administer prescribed medications, treatments, and therapies as ordered by the physician
- Document care provided in a timely and compliant manner
- Provide health education and training to family members and caregivers
- Accept and write verbal orders from physicians as required, and obtain co-signature from the Case Manager
- Follow up with the physician, client, family members, staff, and Case Manager on client problems, concerns, and changes in client condition
- Perform other duties as assigned by the Case Manager
- Active, unencumbered license (RN/LPN/LVN) in the state of practice
- Current CPR certification from the American Red Cross, American Heart Association, or BLS
- TB skin test (current within the last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
- Fingerprinting may be required
