Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate RN to provide continuous care to a medically fragile patient in the school setting. As a School Case RN Home Health Private Duty Nurse, you will be responsible for implementing the physician's prescribed plan of care, conducting patient assessments, and coordinating care with the healthcare team.

Responsibilities:

  • Implement the physician's prescribed plan of care, including administration of prescribed medication, treatments, and therapies.
  • Conduct patient assessments and coordinate care with the healthcare team, including physicians, therapists, and other healthcare professionals.
  • Provide health promotion, teaching, and training to family members and caregivers.
  • Document care provided in the system of record, ensuring compliance with regulatory requirements.
  • Assist patients with travel to and from school, ensuring safe and timely transportation.

Requirements:

  • Active, unencumbered RN license in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within the last 12 months).
  • Six months prior hands-on nursing experience preferred but not required.
  • Reliable transportation.
  • Fingerprinting may be required.
