Licensed Practical Nurse

Position Summary:
- Communicate with nursing staff regarding new patient admissions, including expected arrival times and room assignments.
- Prepare patient rooms for incoming residents.
- Assist in orienting new residents and their families to the facility.
- Accompany physicians during rounds as required.
- Request and coordinate diagnostic and therapeutic services as prescribed by the physician, adhering to established protocols.
- Collaborate with the resident's physician to ensure comprehensive care, treatment, and rehabilitation.
- Review resident charts for specific treatment plans, medication orders, and dietary requirements.
- Implement and uphold nursing objectives and standards as outlined in the care plan.
- Conduct regular checks to confirm that prescribed treatments are being administered correctly by nursing staff and certified nursing assistants, while assessing the resident's physical and emotional well-being.
- Coordinate social and activity programs with nursing schedules.
- Inform the attending physician of any incidents involving residents.
- Notify the physician and family members of any changes in the resident's condition.
- Facilitate restorative and rehabilitative programs, including self-care initiatives.
- Inspect nursing service areas daily to ensure cleanliness and safety standards are met.
- Administer clinical services such as catheterization, tube feeding, suctioning, dressing changes, and other necessary procedures.
- Utilize restraints in accordance with established policies when necessary.
- Ensure that specimens for laboratory tests are collected as ordered.
- Monitor vital signs and document findings as required.
- Provide close observation for critically ill residents.
- Check food brought in by family members to ensure compliance with dietary restrictions.
- Ensure that direct care provided aligns with the resident's care plan and preferences.
- Regularly check on residents who may be unable to call for assistance.
- Engage with residents and their families as needed, reporting any concerns to the Director of Nursing.
- Manage the admission, transfer, and discharge processes for residents.
- Assist in arranging transportation for residents upon discharge.
- Ensure discharged residents are escorted to the designated area.
- Communicate with family members regarding resident deaths.
- Contact funeral homes as requested by families, ensuring adherence to post-mortem protocols.
- Review care plans to verify that appropriate care is being delivered.
- Inform the Care Plan Coordinator of necessary updates to care plans.
- Document nursing care in accordance with the care plan.
- Evaluate resident care plans for relevant goals, issues, approaches, and necessary revisions.
- Ensure that nursing staff and certified nursing assistants are familiar with resident care plans.
- Assist the Resident Assessment/Care Plan Coordinator in the planning and scheduling of assessments and care plan revisions.
Qualifications:
- Must possess a valid Registered Nurse (RN) license.
- On-the-job training will be provided.
