Key Responsibilities
  • Facilitate seamless transitions of care by accepting patient referrals from medical and behavioral health providers.
  • Deliver comprehensive case management services, including specialist referrals and community resource connections.
  • Coordinate care with patients and their families, ensuring that both medical and behavioral health needs are addressed.
  • Utilize a risk stratification model to identify high-risk patients and monitor their progress towards self-care goals.
  • Track and document patient progress, alerting providers when additional services are required.
  • Engage in effective communication with care team members to ensure comprehensive patient support.
  • Provide health education and training to patients and their families, utilizing evidence-based practices.
  • Develop individualized care plans and monitor their effectiveness, addressing any service gaps identified.
  • Participate actively in community meetings and committees to enhance care delivery.
  • Maintain accurate documentation and uphold confidentiality in line with healthcare regulations.
Qualifications

Minimum Requirements:

  • Strong critical thinking and delegation skills.
  • Proficiency in electronic health record systems.
  • Excellent interpersonal and communication abilities.

Education and Experience:

  • Graduate of an accredited RN program.
  • At least two years of experience as an RN in a healthcare environment, with supervisory experience preferred.
  • Experience working with pediatric populations is advantageous.

Licenses and Certifications:

  • Active RN license in the state of Florida.
  • Certification as a Case Manager is preferred.
