As a vital member of our healthcare team, you will work collaboratively with medical providers, behavioral health professionals, and community partners to support patients and their families in achieving healthier lifestyles. Your responsibilities will include assessing patient needs, improving care processes, and promoting patient education regarding both medical and behavioral health issues.
Key Responsibilities- Facilitate seamless transitions of care by accepting patient referrals from medical and behavioral health providers.
- Deliver comprehensive case management services, including specialist referrals and community resource connections.
- Coordinate care with patients and their families, ensuring that both medical and behavioral health needs are addressed.
- Utilize a risk stratification model to identify high-risk patients and monitor their progress towards self-care goals.
- Track and document patient progress, alerting providers when additional services are required.
- Engage in effective communication with care team members to ensure comprehensive patient support.
- Provide health education and training to patients and their families, utilizing evidence-based practices.
- Develop individualized care plans and monitor their effectiveness, addressing any service gaps identified.
- Participate actively in community meetings and committees to enhance care delivery.
- Maintain accurate documentation and uphold confidentiality in line with healthcare regulations.
Minimum Requirements:
- Strong critical thinking and delegation skills.
- Proficiency in electronic health record systems.
- Excellent interpersonal and communication abilities.
Education and Experience:
- Graduate of an accredited RN program.
- At least two years of experience as an RN in a healthcare environment, with supervisory experience preferred.
- Experience working with pediatric populations is advantageous.
Licenses and Certifications:
- Active RN license in the state of Florida.
- Certification as a Case Manager is preferred.
