RN (Per Diem/On Call) - PACE/Elderplace Care Specialist

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Per Diem team at Elderplace PACE in Beaverton, Oregon. As a Per Diem RN, you will provide high-quality care to our patients, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare providers to ensure seamless care transitions.
- Collaborate with the PACE interdisciplinary team to provide comprehensive care, including disease management, health education, and support services.
- Conduct home visits and assessments to identify patient needs and develop strategies for improving functional status and independence.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and effective care coordination.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
