RN (Per Diem/On Call) - Pace/Elderplace

We are seeking a skilled Registered Nurse to join our team at Elderplace, providing per diem/on call support to our Beaverton site and surrounding areas. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our patients.
Key Responsibilities- Provide care management, care coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues.
- Apply population-based disease management approaches and clinical protocols to improve patient outcomes.
- Collaborate with the patient-centered PACE interdisciplinary team to develop and implement comprehensive care plans.
- Assist providers in identifying patient needs and coordinating access to appropriate care and services.
- Travel between multiple locations to provide care and support to patients.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
