We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Per Diem/On Call RN for our PACE/Elderplace program. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients.
Key Responsibilities- Provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
- Develop and implement population-based disease management approaches and apply clinical protocols to improve patient outcomes.
- Collaborate with the patient-centered PACE interdisciplinary team to ensure a systematic approach to care coordination and health services.
- Assist providers in identifying patient needs and coordinating access to appropriate care and services.
- Plan, oversee, and monitor all nursing and unlicensed caregiver services to each patient on their panel.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year of Home Health RN experience
