Nurse Practitioner

We're seeking a highly skilled Nurse Practitioner to join our team at ChenMed, a family-owned and physician-led healthcare organization dedicated to transforming healthcare for seniors. As a key member of our clinical operations team, you will provide direct patient care in our medical centers, working collaboratively with other healthcare professionals to deliver exceptional patient outcomes.
Responsibilities- Assess and manage acute and non-acute clinical problems, performing physical assessments and documenting patient histories, analyzing trends in patient conditions, and developing patient management plans.
- Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying physiological and/or psychological problems commonly encountered.
- Prescribe medication to patients based on State of practice, order and interpret laboratory and diagnostic studies, and refer patients for consultation when indicated.
- Document patient care through in-depth progress notes and summaries, and participate in patient care rounds and conferences to ensure successful patient management strategies.
- ARNP or similar advanced degree in Nursing required, with current Nurse Practitioner Certification in the State of practice.
- Board certification by AANP or ANCC is preferred but may be required for certain States, and a minimum of 1 year of clinical experience preferred.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
