Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at ChenMed. As a key member of our clinical operations team, you will be responsible for providing exceptional patient care to our senior population.
Key Responsibilities- Assess and manage acute and non-acute clinical problems in our geriatric patient population.
- Develop and implement comprehensive patient management plans, including medication orders and referrals to specialists as needed.
- Collaborate with our multidisciplinary team to ensure seamless patient care and transitions.
- Participate in clinical rounds and conferences to stay up-to-date on best practices and share knowledge with colleagues.
- Document patient care and outcomes in a clear and concise manner, adhering to ChenMed's standards for quality and compliance.
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- Board certification by AANP or ANCC is preferred but may be required for certain States.
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
