Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at ChenMed. As a key member of our clinical operations team, you will be responsible for providing exceptional patient care to our geriatric population.
Key Responsibilities- Assess and manage acute and non-acute clinical problems in geriatric patients
- Develop and implement patient management plans in collaboration with the healthcare team
- Provide education and counseling to patients and their families on health and wellness
- Collaborate with multidisciplinary team members to ensure seamless patient care
- Participate in clinical rounds and conferences to stay up-to-date on best practices
- ARNP or similar advanced degree in Nursing required
- Current Nurse Practitioner Certification in the State of practice required
- Board certification by AANP or ANCC is preferred but may be required for certain States
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
