Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our interdisciplinary healthcare team at Vanderbilt University Adult Hospital. As a key member of our team, you will provide comprehensive pharmacy services to internal medicine patients, including developing pharmacotherapy plans, recommending medication regimens, and monitoring patient outcomes.

Key Responsibilities
  • Review and verify medication orders to ensure safe and effective use of medications
  • Provide patient care by optimizing pharmacotherapy plans and resolving medication-related problems
  • Complete pharmacy consults, including pharmacokinetic dosing and medication reconciliation
  • Serve as the pharmacy representative to assigned internal medicine teams
  • Provide drug information responses and manage medication therapy in accordance with federal, state, and local regulations and hospital policy
Preferred Qualifications
  • Completion of an ASHP-accredited PGY1 residency
  • Board of Pharmacy Specialties (BPS) Board Certification (e.g., BCPS, BCCCP, BCCP)
  • Experience working in an academic medical center
  • Experience working with internal medicine teams
Technical Capabilities
  • Medication Management: Demonstrates proficiency in medication management, including determining clinical appropriateness, dose, dosage form, indication, contraindications, potential adverse effects, and potential problems with concomitant medications
  • Pharmaceutical Drug Preparation & Dispensing: Possesses sufficient fundamental proficiency to review, prepare, and dispense prescribed medications
  • Mentoring & Coaching: Constructively trains and mentors others by providing positive direction and feedback
  • Pharmaceutical Computer Literacy: Has demonstrated a proficient understanding of computers or has successfully completed training for applications that support medication use processes
  • Pharmaceutical Quality Assurance & Compliance: Possesses specialized knowledge of rules and regulations and applies them in difficult situations
