We are seeking a highly skilled Transplant Clinical Pharmacist Specialist to join our team at Ascension Saint Thomas West in Nashville, TN. As a key member of our pharmacy team, you will play a critical role in ensuring the safe and effective use of medications in our transplant patients.
Responsibilities- Assure the appropriate selection, dosing, and administration of medication orders in specialized areas, such as nuclear medicine, oncology, and cardiology.
- Provide guidance and support to clinical leadership and pharmacy staff on medication-related issues.
- Review and assess medication orders to identify and prevent potential and actual drug-related problems.
- Recommend formulary alternatives when appropriate and compound and dispense medications for accurate delivery to patients.
- Provide drug information and education to medical staff and patients, and communicate with physicians and other clinicians as necessary to facilitate care.
- Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
- Assist with academic functions, such as serving as a preceptor for pharmacy students, and may float to provide coverage of pharmacy needs.
- Pharmacist credentialed from the Tennessee Board of Pharmacy obtained prior to hire date or job transfer date required.
- Bachelor's or advanced degree in Pharmacy required.
