Job Summary

We are seeking a skilled and compassionate Hospice Field Patient Care CNA to join our team at FirstHealth of the Carolinas, Inc. As a Hospice Field Patient Care CNA, you will play a vital role in delivering high-quality patient care to individuals in their homes.

Key Responsibilities
  • Deliver patient care to hospice patients in their homes, ensuring their physical, psycho-social, cultural, and spiritual needs are met.
  • Perform various tasks, including bathing, hygiene, and ADL needs, in a timely and efficient manner.
  • Work independently, using problem-solving skills to plan and prioritize patient care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate records and reports, adhering to hospice policies and procedures.
Requirements
  • Completion of a NAI Program and current listing with the North Carolina Department of Health and Human Services (DHHS) (N.C. Nurse Aide I Registry).
  • Valid NC Driver's License and CPR certification.
  • Excellent communication and interpersonal skills, with the ability to work in a fast-paced environment.
  • Ability to lift, bend, and stoop, with exposure to blood/body fluids, hazardous materials, and communicable diseases.
Working Conditions

The Hospice Field Patient Care CNA will work in a variety of settings, including patients' homes, and will be required to transport supplies and equipment. The ideal candidate will be able to work in a dynamic environment, prioritizing patient needs and adapting to changing situations.
