Job Summary

We are seeking a skilled Social Worker to join our team at Lifespan. As a Social Worker, you will be responsible for providing social work services to patients and families, assessing their psychosocial needs, and developing plans of care in concert with physicians, nurses, and therapists.

Key Responsibilities
  • Assess patient and family psychosocial needs and develop plan of care in concert with physicians, nurses, and therapists.
  • Link patient/family to appropriate community resources, including information/referral to sources of financial assistance, transportation, food, housing, and support groups.
  • Provide short-term counseling to identify problems and facilitate needed referrals.
  • Serve as patient advocate and liaison with physicians, families, and community to ensure continuity of care.
  • Document patient/family status and history, diagnosis, treatment plan, evaluation results, observations, and progress in medical record.
  • Serve as a resource to other staff on community resources and in area of specialty.
Requirements
  • Master's degree in social work.
  • Additional coursework in area of specialty (e.g. psychiatric, pediatric) as required.
  • Two years experience in social work with broad background with diverse patient populations, preferably in medical setting.
  • Additional one year experience in area of specialty.
  • Eligible for licensure if required by state or position.
Skills and Abilities
  • Knowledge of social work principles and practices.
  • Knowledge of how to use variety of diagnostic tools and techniques, including DSM IV-R.
  • Knowledge of the development and maintenance of information/referral systems.
  • Skill in assessment, diagnosis, treatment, counseling, and care management.
  • Skill in proper use of tools, such as the DSM IV-R, for identifying clinical symptoms.
  • Skill in building and maintaining community resource database by establishing effective liaisons with key sources of assistance for patients and families.
Working Conditions

The employee is frequently required to walk, stand, reach with hands and arms, climb or balance, stoop, kneel, crouch or crawl, and talk or hear. The employee is occasionally required to sit or stand. May be exposed to various patient conditions, including infectious diseases, blood, and bodily fluids.
