We are looking for pediatric hospice social workers to join our team who are committed to creating meaningful experiences for your patients and their families. As a pediatric hospice social worker, you will be responsible for psychosocial evaluations, ongoing counsel, bereavement services and community education outreach. Working in accordance with the plan of care, you will provide physical, emotional, and spiritual support to patients and families when it's needed most.

Essential Functions:
Qualifications:
A heart to serve patients and families and a passion for providing the best possible care
- Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
- Licensure: Current state license as a social worker
- Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
- Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
