Social Worker

As a Social Worker - Patient Care Specialist at Lifespan, you will play a vital role in providing social work services to patients and families. Your primary responsibility will be to assess patient and family psychosocial needs and develop a plan of care in collaboration with physicians, nurses, and therapists.
You will also be responsible for linking patients and families to appropriate community resources, including information and referral to sources of financial assistance, transportation, food, housing, and support groups.
In addition, you will provide short-term counseling to identify problems and facilitate needed referrals. You will serve as a patient advocate and liaison with physicians, families, and community to ensure continuity of care.
Your documentation skills will be essential in recording patient and family status, history, diagnosis, treatment plan, evaluation results, observations, and progress in the medical record.
As a resource to other staff, you will provide information on community resources and your area of specialty.
Requirements:
Master's degree in social work is required. Additional coursework in your area of specialty is also necessary.
Two years of experience in social work with a broad background in diverse patient populations, preferably in a medical setting, is required. Additional one year of experience in your area of specialty is also required.
Eligibility for licensure, if required by state or position, is necessary.
Skills and Abilities:
You will need to possess knowledge of social work principles and practices, staying up-to-date with the latest developments in the field.
You will also need to have skills in assessment, diagnosis, treatment, counseling, and care management.
Your ability to interact effectively as a member of an interdisciplinary healthcare team and use community resource database computer software effectively is essential.
You will need to communicate effectively with diverse patient populations and conduct effective community outreach activities.
Working Conditions:
While performing the duties of this job, you will frequently be required to walk, stand, reach with hands and arms, climb or balance, stoop, kneel, crouch, or crawl, and talk or hear.
You may be exposed to various patient conditions, including infectious diseases, blood, and bodily fluids.

Supervisory Responsibility:
This position has no supervisory responsibility.
