Job Summary

The Care Coordinator for the Healthcare Home Team plays a vital role in organizing the day-to-day tasks of the Healthcare Homes program. This position involves tracking client activity, submitting reports to the Department of Mental Health, and serving as a resource for the Healthcare Homes team and other clinical teams at Burrell Behavioral Health.

Key Responsibilities
  • Provides general administrative support to the HCH Director and Team as needed.
  • Assists in distributing and coordinating reports to the HCH Team, Community Support Specialists (CSS), and community providers when required.
  • Maintains user accounts and monitors agency usage and prescriber reports, promoting the use of Direct Inform.
  • Provides technical assistance to the HCH Team and CSS's on the use of CareManager and CyberAccess.
  • Assists with tracking client screenings and reporting measures, ensuring timely submission as necessary.
  • Attends state-mandated meetings and trainings.
  • Maintains a monthly log of HCH activities and completes the monthly HCH Team Log.
  • Prepares reports for submission to the HCH Director and Assistant Director before submitting to DMH each month.
  • Responds to Quality Assurance procedures, cooperates with utilization review procedures, and adheres to established procedures, rules, and regulations of the Clinical Records Department.
Requirements
  • High School Diploma or equivalent required.
  • Additional Qualifications:
    • Knowledge of chronic health conditions in both youth and adult populations.
    • Excellent written and verbal communication skills.
    • Knowledge of mental health conditions in youth and adult populations.
    • Ability to make decisions based on critical thinking.
