We are seeking a skilled Registered Nurse to join our Homecare team in Plymouth. As a key member of our team, you will have the opportunity to provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.
Key Responsibilities- Provide high-quality clinical services within the scope of practice and in accordance with infection control standards.
- Conduct clinical nursing assessments in accordance with federal and/or state program requirements.
- Ensure patient eligibility and medical necessity for services as defined by payer source and agency policy.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
- Current CPR certification is required.
