About the Role

We are seeking a skilled Registered Nurse to join our Homecare team in Plymouth. As a Registered Nurse, you will play a vital role in providing high-quality care to patients in their homes.

As a member of our team, you will have the opportunity to work in a dynamic and supportive environment, where you can build trusted relationships with patients and their families. You will be responsible for providing and directing provisions of nursing and personal care to patients in their homes, as prescribed by the physician or as authorized by the state/program.

Key Responsibilities
  • Provide high-quality clinical services within the scope of practice and within infection control standards.
  • Complete clinical nursing assessments in accordance with federal and/or state program requirements.
  • Ensure the patient's eligibility and medical necessity for services as defined by payer source and agency policy.
  • Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.
Requirements
  • A minimum of one year experience as an RN is preferred.
  • Must have current RN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification is required.
