As a member of our HouseCalls product, you will conduct annual clinical assessments to identify and reduce health risks for patients, while coordinating appropriate follow-up care to improve health and well-being.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date, with unencumbered licensure.
  • For NPs, active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification; for PAs, Physician Assistant national certification through NCCPA.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home Health care or home visit experience.
  • Ability to travel throughout the state, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
