Key Responsibilities:
- Conduct comprehensive in-home assessments to identify patients' needs and develop personalized care plans
- Perform evidence-based practice screenings, including point-of-care testing, to inform care decisions
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Identify and address health disparities, promoting equitable care and improving health outcomes
- Stay up-to-date with the latest clinical guidelines and best practices to deliver exceptional care
Requirements:
- Active NP or PA license or ability to obtain by start date
- Current national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
- Reliable transportation for home visits
Preferred Qualifications:
- 1+ years of clinical experience in a gerontology, cardiology, internal medicine, or endocrinology setting
- Experience in home health care or home visit assessments
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
