We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Inpatient Heart Failure team at Advocate Aurora Health. As a key member of our care team, you will provide high-quality patient care, work collaboratively with physicians and other healthcare professionals, and contribute to the development of our heart failure program.
Responsibilities- Evaluate and manage patients with acute and chronic heart failure, including those requiring inpatient care and treatment.
- Conduct thorough medical histories, physical examinations, and assessments to determine differential diagnoses and develop treatment plans.
- Prescribe and manage medications, therapies, and treatments as needed, in collaboration with physicians and other healthcare professionals.
- Provide education and counseling to patients and their families on heart failure management, including diet, exercise, and lifestyle modifications.
- Collaborate with the care team to develop and implement individualized care plans, including discharge planning and post-acute care coordination.
- Participate in quality improvement initiatives and contribute to the development of our heart failure program.
- Master's degree in Nursing or Physician Assistant studies.
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Illinois or Wisconsin.
- Board certification as a Nurse Practitioner or Physician Assistant in a relevant specialty.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced, dynamic environment and prioritize multiple tasks and responsibilities.
