Job Summary

A New Leaf's West Valley Housing Assistance Center is seeking a dedicated Case Manager to provide comprehensive support to individuals and families experiencing housing instability.

This role involves conducting assessments, developing personalized case plans, and connecting clients with essential resources such as housing, employment, healthcare, and community services.

The Case Manager will work closely with clients to empower them to achieve long-term housing stability, offering guidance, advocacy, and crisis intervention as needed.

Key Responsibilities:

  • Provide case management services, establish and maintain client files
  • Screen, assess, and develop service plans
  • Develop and monitor the implementation of strength-based case plans and progress towards goals
  • Assist clients in obtaining community services and benefits, securing appointments with outside services and agencies as needed
  • Provide referrals and/or coordinate transportation for clients to community services as needed
  • Facilitate and coordinate discharge planning transition and follow up according to plan
  • Continually improve family cultural awareness and competency
  • Be available and responsive to clients and stakeholders during business hours
  • Complete accurate case documentation and agency paperwork in a timely, correct, and thorough manner
  • Maintain client demographic information and accurate tracking as required by supervisor
  • Provide educational information as needed to assist clients/families in reaching goals
  • Attend and participate in program staff meetings and/or any other meetings as designated by your supervisor(s)

Requirements:

  • Class One Fingerprint Clearance Card
  • Valid Driver's License with reliable transportation
  • Current Auto Liability Insurance
  • CLEAN 39 Month Motor Vehicle Record
  • Must Be At Least 21 Years of Age
