Housing Case Manager

The Housing Case Manager will provide case management and navigation services to program participants at Native American Connections. The ideal candidate will have current knowledge of the Maricopa Regional Continuum of Care (CoC), Coordinated Entry System (CES), experience with housing services in Maricopa County, case management practices, and experience providing case management services to high acuity participants.
Responsibilities:
- Conduct outreach and identify individuals who meet the requirements for the housing program by using the approved assessment tool.
- Ensure participants understand the move-in process, the program guidelines, community rules, case management services, and property rules by the state and federal laws.
- Complete initial Individualized Service Plan (ISP) within 5 days of move-in with SMART goals to ensure housing stability.
- Complete intake forms with participants and evaluate the level of need for workforce and case management services within 5 days of move-in.
- Complete and update ISP and in HMIS weekly; complete at least one in-person case management/home visit per month.
- Deliver and/or facilitate life-skills development curriculum for individuals and groups.
- Connect clients to community resources that offer life skills and employment assistance.
- Conduct monthly housing program meetings with all program participants to address health/safety issues and concerns.
- Address participants' conduct or concerns with participants as needed.
- Provide referrals for emergency medical, behavioral health, and housing stabilization as needed; provide referrals for and/or deliver financial education; take requests and pick up food boxes; manage bus pass program; refer (and follow-up) to DES, applications for food stamps, and AHCCCS as needed; work individually with clients to remove barriers to employability and permanent housing.
- Enroll and terminate participants per agency program requirements.
- Maintain timely, accurate written file documentation on participant attendance to case management services and progress, rental history, medical and BH instances, all referrals, and recertification of income.
- Maintain a shift report in Microsoft Excel that captures interactions with guests, vendors, and outside service providers.
- Schedule and distribute food from the food pantry.
- Issue Guideline notices as needed when Program Guidelines are not followed.
- Participate in weekly staffing by reporting status updates on assigned participants.
- Maintain an Outlook calendar to reflect meetings and tasks.
- Participate in housing-based community meetings approved by the Director of Housing Programs.
- Create and conduct street outreach strategies to target specific areas.
- Maintain outreach reporting of contacts made and outcomes in real-time.
Requirements:
- Bachelor's degree preferred.
- Two years of experience in service delivery or any combination of related education, professional training, or work experience that demonstrates the ability to successfully perform duties.
- Must be comfortable working independently.
- Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
- Must demonstrate critical thinking, problem-solving, organizational, and time management skills.
- Excellent mediation and conflict resolution skills.
- Excellent customer service skills.
- Experience in working with the Native American population preferred.
- Knowledge of the economic, educational, and social problems of Native Americans and referral services.
- Excellent communication skills - written and oral.
- MS Office skills.
- Possess and maintain a valid Arizona driver's license, reliable transportation, and a clean driving record.
- Able to perform general janitorial duties to include bending, lifting, heavy and light cleaning, negotiate stairs, and carry up to 50 lbs.
Indian Preference:
Preference may be given to qualified Native American Indians according to the Indian Preference Hiring Act, CFR 25 USC 472.
