Job Overview

The Case Management role is integral to the mission of Homes With Hope, focusing on delivering essential support to individuals transitioning from homelessness, particularly those with mental health diagnoses. This position involves working closely with clients to ensure they successfully adapt to their new living environments while fostering independence and stability.

Key Responsibilities:

  • Assist a small group of clients in their transition to supportive housing, ensuring they understand and meet their tenancy obligations.
  • Support a larger caseload of clients and families, guiding them through the housing process and helping them adjust to their new circumstances.
  • Conduct thorough assessments to gather relevant information on clients' psychosocial, financial, and health needs, creating tailored Service Plans.
  • Maintain accurate documentation of all client interactions and progress, adhering to required standards.
  • Identify and coordinate access to various services that promote housing success and self-sufficiency.
  • Facilitate referrals to therapeutic, educational, and medical resources as needed.
  • Assist clients in navigating entitlement and benefits systems.
  • Support clients in developing essential life skills, including daily living activities and self-advocacy.
  • Document client progress in compliance with organizational requirements.
  • Participate in ongoing training to uphold professional standards.
  • Provide crisis intervention when necessary, ensuring client safety and well-being.
  • Engage in community outreach and educational workshops to address tenant needs.
  • Build and maintain relationships with community partners and resources.
  • Collaborate effectively with property management to enhance tenant experiences.
  • Perform additional tasks as required to support the mission of the organization.

Qualifications:

  • A Master's degree in social sciences or a related field is required.
  • At least two years of experience in case management or a similar capacity is preferred.
  • Strong teamwork and motivation skills are essential.
  • Excellent communication and interpersonal abilities are necessary.
  • Bi-lingual or bi-cultural candidates are encouraged to apply.
  • Experience working with diverse populations is highly valued.
  • A commitment to the organization's mission of providing compassionate support is crucial.
  • Proficiency in Microsoft Office applications and internet navigation is required.
  • A valid driver's license and access to a reliable vehicle are necessary.
