We are seeking a highly skilled Care Coordinator, Behavioral Health to join our team at CenterWell Home Health. As a Care Coordinator, you will play a critical role in ensuring our members receive the highest quality care and support.

Key Responsibilities
  • Assess and Evaluate Member Needs: Conduct thorough assessments to identify members' physical, environmental, and psycho-social health issues.
  • Develop and Implement Care Plans: Collaborate with members, families, and healthcare providers to develop and implement personalized care plans that address members' unique needs and goals.
  • Coordinate Care Services: Facilitate the coordination of all necessary medical and non-medical services, including functional, social, and environmental services, to ensure members receive comprehensive care.
  • Communicate with Healthcare Providers: Establish and maintain effective relationships with healthcare providers, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination.
  • Monitor and Evaluate Care Outcomes: Regularly monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure members receive the best possible outcomes.
Requirements
  • Master's Degree in Social Work or Therapy: Possess a Master's degree in social work or therapy with a current license to practice in the state of Indiana.
  • Experience in Case Management: Have a minimum of one year of experience in providing case management services to individuals with complex needs.
  • Strong Communication and Interpersonal Skills: Possess exceptional communication and interpersonal skills, with the ability to build rapport with members, families, and healthcare providers.
  • Ability to Work in a Fast-Paced Environment: Be able to work in a fast-paced environment, managing multiple priorities and deadlines while maintaining a high level of productivity and accuracy.
Preferred Qualifications
  • Bilingual or Multilingual: Possess bilingual or multilingual skills, with the ability to communicate effectively with members and families who speak diverse languages.
  • Experience in Behavioral Health: Have experience working in behavioral health, with a strong understanding of the complexities of mental health and substance use disorders.
  • Knowledge of Community Resources: Possess knowledge of community resources and services, including healthcare providers, social services, and support groups.
