Job SummaryWe are seeking a highly skilled and experienced Patient Care Manager to join our team. As a Patient Care Manager, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the provision of quality care to patients, and managing the clinical and administrative aspects of our program.
Key Responsibilities- Manage and oversee clinical and administrative functions, ensuring the delivery of high-quality patient care.
- Respond to phone referrals and inquiries in a timely and professional manner.
- Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
- Manage the site in the absence of the Executive Director, Administrator, or Senior Patient Care Manager.
- Registered Nurse with a minimum of Associate's Degree in Nursing required.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of requested employment.
