The Case Manager will be responsible for providing comprehensive service planning and assessment activities, ensuring service plan implementation through direct case management or co-provider services. This role will also involve providing linkages and advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services, as well as with parole boards.
Key Responsibilities- Participate in orientation of new residents, explaining available clinical services, recreational activities, and ICL programs.
- Obtain and document assessment information, including medical, psychological, and social factors contributing to the resident's situation.
- Collaborate with residents to complete and maintain resident service plans, ensuring consideration of cultural and ethnic background, customs, needs, beliefs, and preferred language.
- Provide supportive counseling in 1:1 or group formats to assist residents in modifying attitudes and behaviors.
- Obtain and develop resource information for residents, disseminating this information in a clear and understandable manner.
- Use cognitive-behavioral techniques to assist residents in meeting service plan goals.
- Provide Critical Time Intervention procedures, as needed.
- Facilitate individualized services to meet the diverse needs of residents and focus on discharge planning goals.
- Perform crisis assessment and nonverbal and verbal crisis intervention.
- Meet weekly with residents to follow-up on service plan goals and objectives.
- Refer residents to community resources and other organizations, accompanying them on scheduled or emergency visits.
- Provide transportation for housing placement, service appointments, recreational, or other service plan-related purposes.
- Assist residents in developing savings and money management plans, optimizing resident income through advocacy with the HRA and VA.
- Plan, escort, and/or arrange social or recreational activities for and with residents.
- Advocate on behalf of residents with outside service providers and within ICL.
- Assist residents in finding and getting into vocational training or other training opportunities, working with other team members to address needs.
- Accompany residents to appointments, as needed.
- Give support and advice to significant others of residents and Transitional Veterans Residence staff.
- Mediate conflicts between residents, negotiating ground rules for relationships.
- Conduct in vivo assessment and living skills training, as needed, accompanying individuals during the first day transition from Transitional Veterans Residence to meetings with community providers.
- Participate in substance abuse services with the substance abuse counselor/specialist to ensure residents have necessary transportation and escort to substance abuse appointments and participation.
- Provide motivational interviewing to residents, as needed.
- Perform regular inspections of residents' living areas to ensure safety, accountability of ICL property, and cleanliness.
- Immediately report serious incidents, allegations, incidents, or sensitive situations to supervisors, completing incident reports in accordance with ICL policy.
- Conduct quarterly reviews of service-plans, assessment information, medical records, and changes in care, following up to determine quantity and quality of service provided.
- Review length-of-stay profile with supervisor, addressing barriers to getting and/or keeping employment and housing, and participating in weekly length-of-stay reviews.
- Participate in ongoing reviews of existing linkages for their adequacy, seeking to extend program and other agency linkages as part of Continuous Quality Improvement.
- Continually document residents' treatment progress, developing measurable and objective service plans that maximize residents' strengths and abilities in accordance with regulatory guidelines.
- Assist in the orientation of new personnel, as requested.
- Maintain up-to-date, accurate individual paper and electronic case records on each assigned resident.
- Comply with attendance and timekeeping rules, reporting reliably and regularly to work on an ongoing basis.
- Attend regularly scheduled clinical meetings, staff meetings, and supervision, as well as in-service training and development activities.
- Review the staff communications log, making entries concerning essential elements of information designated by program management.
- Execute emergency plans as outlined in the policy and procedure manual, administering CPR or first aid to staff or residents who require such services.
- Obtain and maintain CPR and First Aid certifications.
- Obtain and maintain certification as an OD Responder from NYS Opioid Overdose Prevention Program (NYSOOPP) staff.
- May have on-call responsibilities.
- Comply and promote compliance with all applicable laws, regulations, and agency policies, helping to strengthen and maintain an ethical organizational culture.
- Perform other job-related duties, as assigned.
