Job Title: Assistant Case Manager, Behavioral Health Specialist

Caminar is seeking a highly motivated and compassionate Assistant Case Manager to join our team. As a key member of our behavioral health services team, you will provide critical support to clients in achieving their rehabilitation goals.

Key Responsibilities:
  • Assist clients in developing and attaining rehabilitation goals, including securing housing and employment, developing a support system, and increasing socialization skills.
  • Support and assist clients in maintaining personal health, including attainment of and follow-through with medical, psychiatric, and dental care.
  • Teach activities of daily living, such as meal planning and preparation, personal hygiene, and budgeting.
  • Document treatment planning and interventions in a timely manner, according to agency, county, and MediCal billing and quality assurance requirements.
  • Drive agency vehicle to treatment destinations, documenting and reporting mileage according to agency procedures.
Requirements:
  • High School Diploma or GED required; Bachelor's degree in psychology or mental health-related field preferred.
  • Minimum one year of demonstrated work experience with SMI/DD preferred.
  • Excellent computer skills in a Microsoft Windows environment, including email and Internet navigation.
  • Problem-solving, customer service, oral and written communication, quality management, planning/organizing, adaptability, dependability, safety and security, and physical, environmental, and mental requirements.
