As a key member of our hospice care team, you will play a vital role in educating healthcare professionals about our services and providing end-of-life solutions with clinical excellence, compassion, and dignity.
Responsibilities- Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
- Complete the admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions.
- Provide accurate and timely information regarding hospice services to anyone inquiring.
- Receive calls from referral sources, respond timely, and initiate paperwork to begin the intake/referral process.
- Serve as a patient advocate in evaluating/developing a plan of care and obtaining informed consent for hospice.
- Call on multiple account types – SNFs, ALFs, hospitals, and physicians.
- Offer in-services and education on the benefits of hospice.
- A RN license in the state(s) you will be employed.
- A minimum of 2 years of healthcare experience preferred in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Known and respected within your community.
- HCHB experience preferred.
