Job Summary

As a Clinical Team Facilitator with St. Mary's Home Care and Hospice, you will play a vital role in coordinating home health care services for our clients. Under the supervision of the Nurse Leader, you will ensure that all clinical and client needs are addressed in a timely and accurate manner. This includes obtaining and documenting physician orders, responding to incoming and outgoing phone calls, and collaborating with clinical staff to coordinate care.

Key Responsibilities

  • Coordinate home health care services, labs, and equipment for agency clients as prescribed by physicians and/or requested by clients/families.
  • Respond to incoming and outgoing phone calls, physician communication, and document orders in a timely and accurate manner.
  • Collaborate with clinical staff to coordinate care, including scheduling visits and adjusting assignments as needed.
  • Triage operational and clinical calls to/from internal and external customer contacts.
  • Assist Nurse/Therapy Leader and Office Care Manager in ensuring that patients, families/caregivers, and direct care staff are informed of changes to scheduled visits.

Requirements

  • Current Registered Nurse or Licensed Practical/Vocational Nurse licensure in Georgia.
  • Two years or more of home healthcare experience or comparable experience in acute or ambulatory care.
  • Must have current Driver's license and reliable transportation to and from work site.
  • Must be experienced in documenting in electronic medical records and proficient in the use of computer office software.
  • Must possess 1-3 years prior experience in a customer service role.
