We are seeking a skilled Clinical Team Facilitator-LPN or RN to join our team at Trinity Health. As a Clinical Team Facilitator, you will play a vital role in coordinating home health care services, labs, and other equipment and supplies for our agency clients.

Key Responsibilities:
  • Coordinate home health care services, labs, and other equipment and supplies for agency clients
  • Respond to incoming and outgoing phone calls, physician communication, and document orders in a timely and accurate manner
  • Communicate frequently and timely with agency clinical staff regarding clinical care and coordination of care
  • Ensure all clinical/client phone calls and service needs are addressed timely and per policy
  • Obtain and accurately document necessary physician orders for home care services
  • Provide timely follow-up on physician communication regarding lab results, medication/service changes, or other changes in the plan of care
  • Assist office care managers, schedulers, and direct care staff in coordinating assigned patients/visits and adjusting assignments as needed
  • Ensure all care coordination and scheduling tasks are performed accurately and timely, with required information/documentation/scheduling complete and current in the EMR system
Requirements:
  • Two years or more of home healthcare experience or comparable experience in acute or ambulatory care
  • Experience in documenting in electronic medical records and proficiency in the use of computer office software
