Hospital/Clinical Liaison Position Overview

The Hospital/Clinical Liaison will be responsible for the screening of perspective residents for designated facilities in the Elderwood organization. This individual will maintain equal responsibility and loyalty to each designated facility and possess a well-rounded understanding of services, routines, staffing, environment, and general operations within the facilities they represent.

Key Responsibilities
  • Pre-screening of applicants or referrals per established guidelines and policy.
  • Assessing applicants for appropriate placement.
  • Conducting pre-admission interviews of applicants.
  • Obtaining SCREEN form for new residents.
  • Obtaining Level II PASARR examinations for residents, if necessary.
  • Ensuring all necessary documentation required for the admission.
  • Submitting a statistical monthly report to the direct supervisor indicating delineation of screens completed by facility referral, as well as narrative summary of key events.
Requirements
  • Baccalaureate Degree in Social Work, R.N., or related field from an accredited institution required.
  • Experience: 3-5 yrs. hospital patient admissions or related experience.
  • Strong assessment skills and working knowledge of medical charts and terminology required.
  • Valid driver's license required.
